Domain registration is the process by which one registers and reserves the use of a domain name -- the string of characters that becomes a discrete identifiable node on the Internet -- with Internet Corporation for Assigned Names and Numbers (ICANN), which is the organization charged with keeping track of such things and assuring that, for instance, there aren't five different entities all trying to use the same string of characters for five different discrete nodes of Internet.
If you wanted to register "purejuice.org" for instance, what a domain registration would entail would be
a) using a simple utility (domain registrars have these) to search the ICANN databases and determine whether that name was already registered
b) assuming it wasn't registered, registering it to your name (or corporate name) and contact address/phone so that there is some sort of identifiable real-world entity associated with the registration
There are a lot of different companies that work with ICANN, and which offer these domain registration services. There isn't a "one best place," though some offer somewhat lower fees than others. Domain registration is done on a per year basis, with the year running from the day you register the domain.
You do not have to use the same domain registration service to renew a domain registration that you used to register the domain initially, just FYI, so you're not stuck with the one you choose in the beginning.
I have generally used my webhosting company (Dreamhost.com) to do my registrations and renewals. They have been very good to me overall, and I've been with them since 2003.
